Overcrowding and lack of manpower have been highlighted as the main cause of the major breakout from Buimo Jail in Lae, Morobe Province.
This is part of findings by the Commission of Inquiry which was instituted by the Correction Services.
Commissioner Michael Waipo told NBC News, investigations into the breakout have been completed and will be made public soon.
The 2016 Buimo Jail breakout is one of the major jail breaks in the country, where 87 inmates dashed to freedom.
Twelve of them were shot dead.
Commissioner Waipo said the report is before the C-S headquarters in Port Moresby and will be released soon.
